At Home represents a group of organizations with the same ultimate goal of providing the most comprehensive, high quality health care possible to individuals and families. At Home consists of; At Home Health, At Home Palliative Foundation, and At Home at Parkland, a Personal Care Home. The philosophy of our model and care is designed to be able to follow your needs across many skilled levels. Whether we are in your home or you in ours… your care team, with its passion for customer service, will always be by your side.

At Home at Parkland

Our nurses and therapists will provide the care you need without ever leaving home.

Located off 309 North on 40 acres, your home is peaceful and open to the beautiful landscapes. We are a small community with private and semi-private rooms available. All meals and activities are included with our affordable rates. Since we strive on quality care, our rates are not based on needs; rather we base them on the room you choose. All-inclusive rates include personal care items, medication administration, cable, telephone, and peace of mind with nursing staff on-site

Our homes are small by design with only 20 residents living in each building. The smaller community atmosphere allows our care to be the main focus as well as allowing freedom you may not receive at large institutions.

In Home Therapy & Nursing Services

Our nurses and therapists will provide the care you need without ever leaving home.

Skilled services provided in the comfort of your home, assisted living facility, or personal care home. Our teams of Highly Skilled Licensed Professionals make visits to your bedside. For individuals that meet medical necessity, home care services are a wonderful option to keep you safe, healthy, and independent.  Services are covered by all Major Insurance Companies and Medicare/Medicaid. Our group will work closely with your physicians to develop the most comprehensive care plan to address all your needs. The goal of Home Care is to maintain a high level of independence as well as receive superior care. Below is a list of our services we provide.

Services

Skilled Nursing

Skilled nursing is a service that can only be offered by licensed nurses.  This treatment is a higher lever of care.  Skilled nursing includes an array of many complex services including Rehabilitation, would and dressing changes, ostomy and ileostomy care and an ever changing health status.  

 Whether you are receiving our services at home or staying with us at At Home At Parkland we can easily provide most all Skilled Nursing services.  

These services are also covered by most insurances. 

Physical Therapy

Physical Therapy (PT) is a service that only be given by a licensed therapist.  The aim of this service is to reduce pain and improve and restore mobility.  

Our therapists will setup a full comprehensive care plan to help monitor and fully optimize your care.  

At Home Health Care will bring this service right to your door.  Whether you are living independently or here with us at At Home At Parkland.  

This service is covered by most insurances. 

Occupational Therapy

Occupational Therapy is a service that can only be administered by a licensed therapist.  

The purpose for this therapy is to help develop, recover and maintain meaningful every day to day activities.  

Our therapists will develop your own personal care plan to aid in your recovery.  

We will bring this service right to your door, whether you are living independently or living here with us at At Home At Parkland.

This service is covered by most insurances.

Speech Therapy

Speech Therapy is a service that can only be offered by a licensed therapist.  The purpose of this therapy is to help patients with speech, language,  cognitive-communication and swallowing disorders.  

Our therapist with formulate a full care plan for your recovery and maintenance. We will bring this service right to your home living independently or living here with us at At Home at Parkland.  

This service is covered by most insurances.

Medical Social Work

Monitors patient condition by observing physical and mental condition, intake and output, and exercise. ... Assists patients by providing personal services, such as, bathing, dressing, and grooming. Aide services can be covered by some insurance policies or can be set up as a private pay program.

Home Health Aides

Monitors patient condition by observing physical and mental condition, intake and output, and exercise.  Assists patients by providing personal services, such as, bathing, dressing, and grooming. Aide services can be covered by some insurance policies or can be set up as a private pay program.

Specialty Programs

Wound Care and Ostomy Care

At Home Health is considered a Ready Care site for the KCI wound vac, The Gold Standard in wound closure. Our office team will coordinate discharge and delivery of the VAC to the patient and nursing will place as per protocol. Wound care is performed by Wound Care Certified Nurses and have many years of experience treating Surgical, Pressure and Ulcers wounds. We have a licensed Ostomy Nurse on staff that is available to evaluate and plan for Ostomy care and help with issues that may arise. The goal is comfort and the proper fit… and of course to prevent complications.

Disease Specific Pathways: Congestive Heart Failure, Diabetes, COPD

Disease Specific Pathways:
Congestive Heart Failure (CHF): Our program is designed to educate and manage your new diagnosis. CHF is manageable and with the proper tools and direction. Our whole group is involved, Skilled Nursing, Physical Therapy, Nutrition, and your Cardiologist. We will set up a plan and monitor the progress…keeping you independent, safe, and out of the hospital. We have teamed up with the American Heart Association to provide the best tools and outcomes related to your Cardiac Care. Your plan may include diet changes, medication management, lab results and follow-up, as well as physical elements to stay healthy.

Chronic Obstructive Pulmonary Disease (COPD): COPD affects millions of people each day and can cause many side effects that result in loss of independence and confinement to a home or facility. Our pathways help with the complicated disease process and help you develop a good plan that is easy to stick to. Pulmonary Rehab is an important part of the plan as well as working closely with your whole care team. Our Nurses will coordinate this plan with you and keep you ahead of the game.

Diabetes: Newly diagnosed diabetes can be a hard change to your lifestyle. A lot of information all at once is overwhelming, our Team of Nutritionist, Nurses, and Therapist will lead you through the process. Diabetes is a lifestyle change and with the right tools and direction can be very easy and enjoyable to manage.

Joint Replacment Surgical Program

Joint Replacement Program: Our Joint Venture Pre and Post Surgical program was developed by our Nurses and Physical Therapists over the years to give the best outcomes and rehabilitation you need to get back to your life. The idea of the program is to have you meet or talk with your Nurse and Therapist before you go to the hospital. This is not only a good time to have all your questions answered, but also a great way to meet your team before you go to the hospital. Once in the hospital our office will follow your progress with the Case Manager and be ready for your discharge. The same team will then see you at your home to start Rehabilitation and the Nurses will monitor pain, surgical wound, and medications. We always work in conjunction with your surgeon keeping them informed of your progress and any issues that may come up. Our staff is on-call 24 hours a day to help and assist. The program is designed for 4-6 weeks of care and is covered by insurance.

Palliative Care

Palliative Care is a philosophy with the focus of symptom management and curative care. Individuals faced with life limiting illnesses need additional attention to plan for the near future with medication management, treatment options, and emotional support for everyone involved. Palliative Care works best when started early in the diagnosis process by allowing the group to make a comprehensive care plan and develop a relationship to continue through treatment. The Palliative Team works with your current physicians and offers more support to family and friends. Palliative Care is covered by Most Insurance Companies as well as Medicare.
